The HAWKS Board of Directors is committed to creating the most positive experience possible for our student-athletes. The kids work very hard in school and on the field and deserve our best effort.  Here is the latest information to help you register your child or for you to register as a coach for the 2008 season.

 

 

 

 

2008 Registration Information/Instructions for Participants

  • Equipment and uniforms are limited to 30 children per team.  Registration is done on a first-come, first served basis.  Since teams fill up quickly, please submit your completed registration paperwork as soon as possible.
  • Registration deadline has been extended until May 4, 2008.
  • All paperwork requiring a parent’s signature must be signed by a parent or legal guardian of the child. 
  • Full refunds available until August 15, 2008.
  • Mail-in Registration is highly recommended for football and cheer.  Please include a check for the full registration fee along with all Mail-in Registration paperwork.  Please send all Mail-in Registrations to:

  • Required Paperwork to make things easy the only paperwork you will need to register your child by Mail-in or Walk-in Registration is the 2008 Registration Form.
  • 2008 Registration Form: This one-page document should be completed and submitted by Mail-in or at Walk-in Registration.

All other forms will be collected no later then the first practice by your head coach or team parent, as follows:

  • Parent’s Consent Form: This one-page document provides us with the legal considerations, consents, approvals and releases required to permit each child to be actively involved in Milford Youth Football and Cheer Inc. program.
  • Birth Certificate: For first time participants, please provide us with an original, raised seal birth certificate, plus a clear copy.  Returning children who are already certified do not need to submit a birth certificate.  If your child skipped one or two years in the program you maybe asked to resubmit an original, raised seal birth certificate, plus a clear copy to recertify them. 
  • Parent Participation Form: Milford Youth Football and Cheer Inc. run 100% on volunteers who give their time generously to help the kids.  We require each family to volunteer and help their team throughout the season.   
  • Code of Conduct Form: This one-page document reinforces to everyone the importance of following good sportsmanship rules when participating in youth sports. 
  • Photo Release Form:  This one-page document permits Milford Youth Football and Cheer Inc. to photograph and/or video your child during games, practices and other organization sanctioned events and publish the image, photograph or video on our web site or in print media (newspapers) for recognition or promotional purposes.  Milford Youth Football and Cheer Inc. believe in child safety, children photos will not be identified by name without additional permission.

 

 

  • Doctor’s Consent Form:  This one-page document must be signed and dated by your doctor after January 1, 2008.  No player will be permitted to practice or be with the team prior to the head coach receiving the Doctor’s Consent Form.  If your doctor refuses to use this form, please have them fill out a written consent or put on their form used for physical that the child is clear to play football or cheerleading. 
  • Final Report Card:  Please submit a copy of your child’s final report card from the most recently completed school year.  A new copy is required each year.
  • Any missing information or forms by the above mentioned dates will be cause for that Participant not to be registered.  If the required paperwork is not given to the head coach by the first practice the player will be sidelined until the parents submit the required information.  Other consent or Code-of-Conduct Forms or contracts may be required at a later date if approved by the Milford Youth Football and Cheer Inc. Board of Directors.  The Board of Directors will make every effort to communicate the timelines and requirements if anything changes as the season nears.  Please make sure you always check the Milford Youth Football and Cheer Inc. website at www.milfordfootball.org to keep current. We look forward to a fun and successful 2008 season.
  • Refund Policy:  Refunds will be distributed in August.  To receive a refund complete the Application for Refund Request Form located on this website and e-mail it to Skip Dicapua at pacifico@decapualaw.com.  No refunds will be given after August 15, 2008.
